We use protected health information (PHI) to provide treatment, obtain payment for services, and run our practice (health-care operations). We may share PHI with other clinicians involved in your care.

- Request amendments to your record

- Receive an accounting of certain disclosures
- Receive a paper copy of this notice on request
Our obligations
We are required by law to maintain the privacy of your PHI, provide this notice, and notify you following a breach of unsecured PHI. We must follow the terms of the notice currently in effect.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
